Pet Adoption Application

Thank you for your interest in providing a lifelong home for one of our dogs/cats. The completion of this questionnaire is the first step towards what we hope will be many years of happiness for both you and our rescue dog/cat. Once you’ve answered all the questions, a volunteer will review your application which may take 1 – 2 days and may also have follow-up questions to ask.

Please understand that we want the best fit for you and the rescue dog/cat.

Completing this process does not guarantee an adoption approval. We want the best match of personality, temperament, and home environment for both the adoptive family and the dog/cat. We are looking out for the best interests of our animals. We are the only voice they have.

If your application is approved, you will be asked to sign a legally binding contract when adopting your dog/cat.

Conditions of Adoption:

·      Valid identification

·      If renting, copy of lease that states pets are permitted.

·      Ability and willingness to spend the time and money necessary to provide training, medical treatment and proper care.

·      Willingness to provide a lifelong commitment to the care and well-being of your pet(s).

·      In-home visit with family, pets, and the prospective adoptive dog/cat

While it is our intention to find forever homes for our animals, we do recognize that some unforeseen events occur that cause the need to give up a cherished pet. In that event, we require that you return the animal to us.

This saves you from the stress of trying to find a new home for your pet, and it saves us from the heartbreak of finding out one of our rescues is in danger of being euthanized at a shelter or placed in an unapproved home not familiar with our policy.

For this reason, we include a clause in the adoption contract that you return the animal to us should you not be able to keep him/her regardless of the reason. Our goal is to find good homes for our animals, please trust us to do so.

By signing/typing my name on this form, I acknowledge that I have read and understand the contract and return policy of Companion Animal Care of Brevard County, Inc.
